About Keep Well
Keep Well is an example of anticipatory care in practice, developed as part of plans to tackle health inequalities in Scotland. The programme aims to increase the rate of health improvement in 45-64 year olds in areas of greatest need. There is a particular focus on early intervention for those at a high risk of coronary heart disease and diabetes.
The intention is to further develop primary care services to deliver anticipatory care, and where appropriate link with other partner agencies. This approach involves:
- identifying and targeting those at particular risk of preventable serious ill-health (including those with undetected chronic disease)
- offering appropriate interventions and services to them
- providing monitoring and follow-up
